Three Meadows Post Acute nursing facility is located in Perrysburg in Ohio. We’ve served the community since 1980. We have in-house rehab therapists and state-of-the-art gym.

Leadership Team

Our Licensed Nursing Home Administrator (LNHA) directs all facility operations, staffing, services, and programs. The leadership team includes the Medical Director, who coordinates medical service.

Nursing & Clinical Staff

The Director of Nursing leads the nursing staff. They are responsible for delivering care and clinical services. Registered Nurses (RNs) assess residents, plan, provide, and document nursing care.

Therapy/Rehabilitation Team

Our licensed Physical Therapists (PTs), Speech Therapists (STs), and Occupational Therapists (OTs) create and implement therapy plans. The Rehabilitation Director leads the team and guides strategies.

Social Services & Activities

The Social Services Director leads and plans programs that address the mental, social, and emotional needs of patients and families. Social Workers work under the director and provide services.

Dietary Services

Registered Dietitians (RDs) oversee meal planning and presentation. This team ensures our residents receive nutritious and appetizing meals that meet their needs.

The Environmental Services team oversees facility maintenance, laundry, general housekeeping, and cleanliness. We abide by local, regional, and national health and safety standards.
